TikTok Shop Adds Toddler OTC and Tightens Cough Rules

Starting October 2, 2026, TikTok Shop tightened rules for cough syrups and added an invite-only toddler category. Dextromethorphan is now the only permitted cough ingredient and only certain states can receive it. Review your listings now.

What changed

• Added an invite-only Toddler OTC category that is high risk. Only acetaminophen and ibuprofen for pain/fever, plus antacids and simethicone for digestion, are allowed.
• Introduced an ingestible cough relief category that only allows dextromethorphan as the single active ingredient. Allowed formats are syrup, oral solution, lozenge, tablet, or capsule.
• Packages must not exceed 900 mg of dextromethorphan, and orders are capped at two units. Shipping is restricted to a specific allowlist of states; all others are prohibited.
• Banned items now cover combination cough and cold remedies with multiple symptoms, decongestants, allergy meds, and any toddler cough or cold product.
• For the toddler category, only invited, vetted healthcare companies or their authorized resellers can apply. They must upload all label sides, a regulatory pathway statement, supply chain evidence, and a recent COA from the last 180 days.

Who it affects

US sellers with OTC listings, particularly cough syrups or toddler items, need to check new restrictions. If you ship dextromethorphan from a non-allowlisted state, you must stop. Only established healthcare brands with an invitation can enter the toddler space.

What I would do this week

• Review your current catalog for ingestible cough products. Remove any with more than one active ingredient and delete toddler cough, cold, or allergy listings immediately.
• For dextromethorphan-only cough items, verify the active ingredient list, keep package totals at or below 900 mg, and restrict shipping to the approved state list.
• To sell toddler pain or digestive meds, ask your Account Manager if you qualify for an invitation as a healthcare brand or authorized reseller.
• Gather unedited photos of the full label, a completed regulatory pathway form, proof of supply chain, and a test certificate no older than 180 days.

My honest take: this update makes OTC medication selling far more restricted. The toddler category is not worth pursuing unless you already have healthcare credentials and FDA paperwork in order. The dextromethorphan state allowlist will create shipping logic problems for smaller sellers. If in doubt, pull the listing and wait for clearer guidance from TikTok Shop.
